NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in every living cell, essential for converting food into cellular energy and regulating hundreds of metabolic processes.
Core roles:
Energy metabolism: NAD+ shuttles electrons in the mitochondrial electron transport chain, driving ATP production during glycolysis, the TCA cycle, and oxidative phosphorylation. It cycles between its oxidized form (NAD+) and reduced form (NADH).
Sirtuin activation: NAD+ is the required cofactor for sirtuins (SIRT1-7), a family of enzymes that regulate gene expression, DNA repair, inflammation, and mitochondrial biogenesis. Without adequate NAD+, sirtuin activity drops.
DNA repair: PARP enzymes (poly-ADP-ribose polymerases) consume NAD+ to repair damaged DNA. Heavy DNA damage can deplete cellular NAD+ pools since PARPs compete with sirtuins for the same substrate.
Immune signaling: CD38, an NADase expressed on immune cells, is a major NAD+ consumer and increases with age and inflammation, contributing to age-related NAD+ decline.
NR bypasses the rate-limiting step of the de novo pathway (tryptophan → quinolinic acid → NAD+), which is inefficient in humans.
Unlike nicotinamide, NR does not inhibit sirtuins at high concentrations (nicotinamide is a known feedback inhibitor of SIRT1-3).
NR crosses cell membranes via equilibrative nucleoside transporters, giving it reasonable oral bioavailability compared to NAD+ itself, which cannot cross cell membranes intact.
Transporter mechanism: NR crosses cell membranes via equilibrative nucleoside transporters (ENTs), a well-characterized transport route. NMN requires dephosphorylation to NR before cellular uptake.
Molecular size/charge: NR lacks the phosphate group NMN carries, which supports the assumption that NR passes through membranes more readily.
Human clinical data volume: NR has a larger body of published human trials (dose-finding, safety, pharmacokinetics). NR holds GRAS (Generally Recognized As Safe) status in the US, while NMN's regulatory status has been obscure.
Niagen NR raises blood NAD+ in a clear dose-dependent pattern: 100 mg/day gives about a 22% increase, 300 mg/day about 51%, and 1000 mg/day about 142%. Levels plateau around two weeks and hold steady with continued daily use.Separately, trials administering NR up to 2000 mg/day (usually in split doses, twice daily) for up to 12 weeks have shown it's safe and effective at raising NAD+ at a higher range — though no study has yet directly compared once-daily versus split dosing, or whether time of day matters.

Most labs measure NAD+ on a relative scale. That's useful for comparing one sample to another, but it doesn't give you a standard number like LDL cholesterol, blood pressure, or heart rate.
The Brenner laboratory developed a more precise method called quantitative targeted NAD+ metabolomics. It uses tandem mass spectrometry to measure more than a dozen molecules involved in making and breaking down NAD+, all on a molar scale. Because everything is measured in the same units, scientists can directly compare different NAD+-related molecules to each other, regardless of their size.
Even with this advance, NAD+ testing hasn't moved from the research lab into everyday clinical use. Two things are still missing: a standardized method that clinical labs can adopt, and an established "normal range" for the general population. Both will take more research.
Regarding tissue NAD+. Every organ makes and clears NAD+; a blood NAD+ reading doesn't necessarily reflect what's happening in tissues such as the liver, heart, or brain. Blood is non-invasive and works well for tracking a patient's response to Tru Niagen. Since no one is signing up for liver or brain biopsies, researchers are actively working on less invasive ways to assess NAD+ status in the tissues that matter most.
No — nicotinamide riboside does not cause the flushing reaction associated with niacin (nicotinamide/niacinamide's cousin, nicotinic acid).
NR is metabolically distinct. It converts to NAD+ through the NRK1/NRK2 → NMNAT pathway never passing through the nicotinic acid route that triggers GPR109A activation. Since NR doesn't touch that receptor, it doesn't produce the vasodilation and prostaglandin release responsible for flushing.
Human trials of NR, including at higher doses (1000-2000 mg/day), have not reported flushing. This is one of the practical advantages NR has over niacin for practitioners who want to raise NAD+-related pathways without the tolerability issues that often cause patients to discontinue niacin therapy.
